FOR YOUR APPOINTMENT at the John C. Hart Memorial Library, you must fill out an application, provide proof of U.S. citizenship, proof of identity, one passport photo which can be taken at the library, and pay the applicable fees. “Processing Times” can be found on the U.S. Department of State website at: Need a Passport (state.gov)
Fill out an application for passport, Form DS-11
Please complete the form legibly in BLACK ink or print the form from the travel.state.gov website. DO NOT sign the form. You will be asked to sign this form in the presence of a passport agent.
Provide proof of U.S. citizenship
Your evidence must be an original or certified, physical copy. A certified copy is any document that has the seal or stamp of the official issuing authority. You must submit one of the following documents:
- Full validity, undamaged U.S. passport (expired passports are accepted). Full validity means the document is/was valid for 10 years for adults and 5 years for children under 16.
- U.S. birth certificate that meets the following requirements:
- Issued by the city, county, or state of birth
- Lists your full name, date of birth, and place of birth
- Lists your parent(s)’ full names
- Has the date filed with registrar’s office (must be within one year of birth)
- Has the registrar’s signature
- Has the seal or stamp from the city, county, or state which issued it
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization
- Certificate of Citizenship

Provide Proof of Identification
You may prove your identity with:
- Valid or expired, undamaged U.S. passport book or passport card
- In-state, fully valid driver’s license or enhanced driver’s license with photo
- Certificate of Naturalization
- Certificate of Citizenship
- Government employee ID (city, county, state, or federal)
- U.S. military or military dependent ID
- Current (valid) foreign passport
- Matricula Consular (Mexican Consular ID) – commonly used by a parent of a U.S. citizen child applicant
- U.S. Permanent Resident Card (Green Card) – commonly used by a parent of a U.S. citizen child applicant
- Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards)
- Enhanced Tribal Cards and Native American tribal photo IDs
- Note: you may be asked to present an additional ID when presenting one of these three documents: 1) In-state, fully-valid learner’s permit with photo, 2) In-state, fully-valid non-driver ID with photo, or 3) temporary driver’s license with photo.

Provide one passport photo
Please provide one 2”x2” full face color photo, taken within the last six months. Photos need to show a current appearance, exhibit a clear front view of your entire face, and have a white background. You should not be wearing a hat, headgear or glasses in the photo. For details see U.S. Passport Photos (state.gov).
You may bring in a photo or get a photo taken at the library for $10.00.
Fees
Different fees are collected when applying for passports. All fees must be paid by check or money order only and payable to the US Department of State.
- Passport BOOK: Adults (16 & above) = $130.00
- Passport CARD: Adults (16 & above) = $30.00
- Passport BOOK: Child (under 16) = $100.00
- Passport CARD: Child (under 16) = $15.00
- Expedite Fee: Additional $60.00, plus $18.32 for 2-day return
Processing fees for each passport application may be paid for in cash, a separate check, or money order payable to the John C. Hart Memorial Library:
- Passport Execution Fee: $35.00
- Overnight mail TO Department of State: $27.90
- Passport photo services taken on-site: $10.00
Requirements for minors
All children, including newborn infants, must have their own passport. In addition to complying with the general requirements for applying for a passport. Visit the Westchester County Clerk Page to learn about the additional requirements for children applying for a U.S. Passport.
